Good Faith Estimate – No Surprises Act (NSA)

As of January 1, 2022, all health care providers are required to provide an estimate of anticipated charges prior to providing services to clients or patients who do not have insurance or who are not using insurance (self-pay). This is referred to as a “good faith estimate.”

If this applies to you, Dr. Schloesser will provide an estimate of anticipated costs prior to your first scheduled appointment. The estimate will include rates of services as well as the projected number and frequency of sessions, a description of the services provided, applicable diagnosis codes, and provider identification information. Please note that until an intake has been completed, Dr. Schloesser may not identify a specific diagnosis. The information provided in the good faith estimate is an estimate, and the actual items, services, or charges may differ from what is included in the good faith estimate. The estimate will be updated as needed. A good faith estimate is for your information. It does not commit you to receiving services.

To learn more about the Good Faith Estimate, you may go to www.cms.gov/nosurprises or call 800-985-3059. If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the bill by going to www.cms/govnosurprises. There is a $25 fee to use the dispute process.
